OACH & SC provides Community Support Services and assist with locating a suitable place to call home for Adults, Families, Transitional Aged Youths, and Individuals Re-enterying the community from incarceration that are experiencing homelessness and or suffering from a mental disorder . OACH & SC has developed a housing program that is designed to provide immediate access to Emergency Shelter, Transitional Housing , Shared Living , Permanent Housing Navigation and Recuperative Care for those rehabilitating. Our Housing Coordination Team works as an advocate on behalf of our participants to assure that the participant obtains Safe , Secure and Affordable housing.OACH & SC also provides housing sustainability services to help prevent eviction. OACH & SC collaborates with Local and County programs to connect participants to other housing programs and benefits.
