Objective
Open Arms Community Health and Service Center provides access to quality Healthcare , Mental Health and Supportive Services. OACH&RC works with Adults and Youth that are in need of Primary Care, Mental Health Services, Substance Use Treatment, Recuperative Care, Emergency Housing, Permanent Housing Navigation, Counseling, Groups, Workforce preparedness and programs to help the Justice Impacted transitioning into the community.

Mission
It is Our Mission along with our Physicians and Mental Health Specialist to provide a safe place within the community where services and support are provided to anyone in need. To reduce the amount of people that suffer from poor health and do not have access to Mental Health and Substance Use treatment. To prevent homelessness by providing housing stabilization services and help those who are homeless secure a safe place to live.

We aim to identify the needs of our participants and develop a patient-centered plan to stabilize our participants health , mental health and housing needs. We are dedicated to supporting our participants and helping to redirect there path into being self sufficient and living a healthy life style.

Our Care Coordination Team works together to ensure that participants receive the best medical treatment and are connected with supportive services to enhance the patient treatment plan and resource linkage. Our Care team is here to help provide services until the participant has successfully reached their goals.
