Overview:
The Lead Care Manager facilitates transition of care planning with an interdisciplinary healthcare team to achieve quality outcomes, reduce costs and ensure patient safety and treatment adherence.
Qualifications:
Education-
-Graduate of an accredited School of Nursing, or Licensed Physician Required
Experience
2-3 years acute care hospital setting, Required
Care Management or Utilization Management experience, Preferred
Service Assessments / Intake,
Patient Engagement & Care Planning,
Motivation Interviewing,
Health Promotion & Education,
Care Coordination,
Other Skills & Knowledge :
Strong verbal and written communication and facilitation skills,
Strong assessment skills, time management skills, critical thinking skills,
Advanced computer skills,
Must have ability to interact effectively with a variety of people and situations at all levels of the organization, Minimum Required

Assessments to develop a Individualized Care Plan (ICP)
Care Coordination
Enagaing eligible members
Oversea implementation of Care Plan
Monitor Treatment & Medication Adherence
Provide Health Promotion and Self Management
Advocate on behalf of member with Heath Care providers
Offer services where the member lives, seeks care, or finds mostly accessible.
